Rates:
$170 for Initial 60 minute evaluation at first session
$150 per 55-minute individual session thereafter
$170 per 55-minute session with partner/family in session or family/partner without client in room (as needed)
Insurance and Payments:
Major Credit Cards and Health Savings Account Debit Cards are accepted and
collected at first visit. They are saved on a HIPAA-compliant format
electronically. The cards are charged at each visit for co-pays,
co-insurances and private pays. The cards are also charged for late cancels
(giving less than 24-hr notice) and "no-shows."
I am an in-network provider with Premera, Lifewise,
Tricare (recognized by some plans), and Kaiser PPO (not in-network with "core or HMO" but you may be able to get an authorization for in-network services).
*When I am an Out-of-Network Provider, I will provide
forms for you to bill your insurance for reimbursement.
**It is the client's responsibility to verify mental health coverage. With
some insurance plans, you may be required to get an authorization code
before your first counseling session. Some insurance plans involve other
insurance plans to cover mental health services. In those cases, I may or
may not be in-network with those other agencies. Check with your insurance
to see if they recognize me as an "in-network mental health outpatient
provider" before deciding to make an appointment, or you may be
responsible for whatever the insurance does not cover. My NPI is

*Because I am located in WA state, insurance billing will go through that state. All OR clients will be telehealth.
Some helpful questions you can ask your insurance :
- What are my mental health benefits (deductible, co-pay or co-insurance, how much deductible has been met, etc)?
- What is the coverage amount per therapy session?
- How many therapy sessions does my plan cover?
- How much does my insurance pay for an out-of-network provider?
- Is approval required from my primary care physician?
- (If I am out of network) Do I need to give permission for Denise Hie to speak freely with my insurance provider about financial-related developments?
Cancellation Policy:
If you do not show up for your scheduled therapy appointment, and you have
not notified me at least 24 hours in advance of a cancellation, you will be
required to pay the full cost of the session. Your insurance will not pay
for it, and they will not apply it to your deductible. Your card on
file will be charged on the appointment day.
